They don't need have any of the 'traits', per se, if by which you mean physical, to count as trans. Though I think they generally seek to make those traits part of them to better represent how they feel. Hence the clothes they wear, lifestyle choices, gender reassignment, hormone therapy. The extent to which they go of course depends on the person, once again.

So no they don't need to look like Saika.

I mostly meant personality traits. If it's as simple as just identifying yourself as one I guess that explains why it's so prevalent on sites like tumblr.

I always love people that say they "avoid labels". It's impossible to avoid labels, labels are part of the human condition! It's how your brain makes sense of things! Labels are a part of life! It's normal! 

Yep, but I don't think it's out of date. There may be a few medical things that may be out of date, but it's probably very minor.

Then again, it's extremely rare to actually have a "true" hermaphrodite anyways. It's usually just ambiguous external genitals caused by a hormonal defect.
